What Is Erectile Dysfunction?

How Do Erections Work?

In order to understand erectile dysfunction, it’s important to understand how erections are supposed to work.
Looking at this illustration, you can see that the penis is actually made up of of 3 separate tubes. There is a tube that carries urine, known as the urethra (the red tube on the bottom), and two tubes that provide erections (the two purple tubes). These are known as the corpora cavernosa. When a man is stimulated, an electrical signal from the brain travels down the spinal cord, through the nerves of the pelvis that attach to the prostate, to the penile arteries (the red dots inside the purple cylinders). The penile arteries then dilate, becoming many times larger than they are at rest. This allow a large rush of blood that cause the corpora cavernosa to expand and become more rigid, blocking the veins that normally drain the blood from the penis and providing an erection sufficient for intercourse. Although this mechanism may sound simple, it’s actually quite complex!

How Can Things Go Wrong?

The penile arteries are unique in that they are the smallest functional arteries in the human body that don’t have a ‘back-up’. This means that when there’s a problem with a man’s penile arteries, there’s going to be a problem with his erections. As men age and begin to collect more and more ‘miles on the odometer’ the penile arteries lose their flexibility and ability to dilate. This problem can be made worse by a number of common medical issues including diabetes, heart disease, high blood pressure, and smoking.

Damage to the nerves that supply erections can also cause erectile dysfunction. This is mostly commonly seen in men who have undergone treatment for prostate cancer. The nerves that supply erections are directly attached to the prostate and are frequently injured or stretched when the prostate is removed or treated with radiation. This can be devastating to men who have beaten cancer only to be faced with the prospect of losing their sex life and intimacy with the ones they love.

As erectile dysfunction progresses, the penis enters what is known as a ‘hypoxic state’. This means that the sensitive tissues responsible for erection are denied the oxygen that they need to remain healthy. This can cause progressive scarring or fibrosis of the erectile tissue. This results in a progressive loss of both length and girth over time! Although this process is difficult to reverse, the sooner men seek treatment, the more penile size can be preserved or restored.
